This form is a direct selling contract. You may cancel it within 10 working days of signing, for a full refund — see Part G. You do not have to buy anything to join, and membership is free.

You must be at least 18 years old and lawfully able to engage in business in the Philippines. One person may hold only one membership account; spouses may share one joint account.

Bonuses are paid in Philippine pesos at 1 point = ₱80.00, to an account in your own name only. The Company withholds tax as the Bureau of Internal Revenue (BIR) requires and gives you BIR Form 2307 for the tax withheld.

Part E – Declarations

I do not already hold a membership account with the Company, and I have declared my spouse's account if they hold one.

I have been given, and have had the chance to read, the Distributor Agreement, Code of Ethics, Compensation and Marketing Plan, Returns, Cooling-off and Buy-Back Policy and the Privacy Notice.

I understand that the Company does not guarantee any income, that bonuses come only from product sales, and that nothing is paid for registering or sponsoring members.

I will not buy products I cannot use or resell within a reasonable time in order to qualify for a rank or bonus.

I will not make medical or therapeutic claims about any product, or claims about income.

Part F – Personal data (Data Privacy Act of 2012)

YESGLOBAL NUTRITION & WELLNESS INCORPORATED is the personal information controller for the data in this form. We use it to assess this application, verify your identity, keep your member account, calculate and pay bonuses, withhold and report taxes, deliver your orders and meet our legal obligations. Your sponsor and upline see your name, member ID and the sales volume of your organisation; they never see your government ID, TIN or bank details. The full details, including your rights and how to contact our Data Protection Officer, are in the Privacy Notice.

I have read the Privacy Notice and I consent to the processing of my personal data, including my government ID number and TIN, for the purposes above.

Optional: I also agree to receive news about products, promotions and events. I may withdraw this at any time.

Part G – Your right to cancel

You may cancel this contract within TEN (10) WORKING DAYS of the date you sign it. You do not have to give a reason. The Company refunds everything you paid, including any first purchase, within fourteen (14) days of your notice. Working days exclude Saturdays, Sundays and public holidays. Notice takes effect on the day you send it. Use the Notice of Cancellation below, or write to ph@yesglobal.co. This right is given by the Company and is in addition to your rights under the Consumer Act of the Philippines (Republic Act No. 7394).

Part H – Signature

By signing, I apply to become an Independent Distributor and accept the Distributor Agreement, Code of Ethics, Compensation and Marketing Plan, Returns, Cooling-off and Buy-Back Policy and the Privacy Notice, each as Version 1.0. My cooling-off period starts on the date I sign and does not depend on when the Company accepts this application.
